Collaborative Family Healthcare Association 14th Annual Conference

The Future of Integrated Healthcare: Activating Clinicians, Consumers, Researchers, and Policy Makers

October 4-6, 2012 Austin, Texas

The Collaborative Family Healthcare Association (CFHA) is a member-based, member-driven collaborative organization. For us, collaboration isn’t just a word in our name; it defines who we are, how we interact with each other and other organizations. We believe deeply that collaboration is an essential element necessary for re-visioning healthcare specifically and society generally. CFHA promotes a comprehensive and cost-effective model of healthcare delivery that integrates mind and body, individual and family, patients, providers and communities. CFHA achieves this mission through education, training, partnering, consultation, research and advocacy.

The CFHA Annual Conference focuses on the many and varied aspects of collaborative and integrated healthcare. It has become the premiere conference of its type, bringing together practitioners, researchers, students, academicians, consumers and patients, corporations, governmental agencies, foundations, and many others.

More than 400 attendees are expected to attend the 2012 CFHA Conference in Austin, Texas. The three-day conference includes preconference workshops; three plenary sessions and more than 50 educational breakout sessions.

The theme of CFHA’s 2012 conference is The Future of Integrated Healthcare: Activating Clinicians, Consumers, Researchers, and Policy Makers. In Texas as well as nationally, building capacity to increase access, improve outcomes and maximize efficiency in healthcare are essential to meeting the population health and economic imperatives before us. Together, we can create the opportunities in policy and practice, share the learning, and combine our voices to make collaborative care the standard rather than the exception. With that as our goal, we must leverage our creativity and our expertise in research, policy, practice, education and training, and advocacy to fuel the movement and reach the tipping point.

This conference is your opportunity to share your expertise and your creativity with others from whom you too can learn. This is an opportunity to expand connections and accelerate our collective capacity to become effective change agents in the adoption of collaborative care.
